Liability: in consideration of being permitted to participate in any form of training with Motivational Fitness CT LLC the creator of The 30 Day Trilogy phase 1 or instructor(s), I agree to assume full responsibility for any risks, injuries, damages known or unknown, which I might incur as a result of participating in any form fitness or combative training. In further consideration of being permitted to participate in any form of fitness or combative training with Motivational Fitness CT LLC or instructor(s), I knowingly, voluntarily and expressly waive any claim I may have against Motivational Fitness CT LLC or any instructor(s) for damages, injuries including death that I may sustain as a result of participating in any form of fitness or combative training. I, my heirs or legal representatives forever release, waive, discharge and covenant not to sue Motivational Fitness CT LLC or instructor(s) for any injury or death caused by voluntary participation in any form of fitness or combative training.

Release: I have read the above release and waiver liability, policies for The 30-Day Trilogy training program and fully understand its contents. Furthermore, I certify that I am in good physical health. If I am being treated by a physician or am on medication, I have consulted with my physician before beginning the program.

I have read and I am in agreement with all that was aforementioned with the purchase of The 30-Day Trilogy.
